What this page tells you

At the 7T scan, bring two consent copies, 7T C2, W9, metal screening and demographic form; fill the REDCap 7T MRI and consenting fields. For controls, trigger the control check to Joel after Gabby uploads to PennChart. Includes the 7T consent-note template.

Consent notes
This patient was consented to the Davis 7T protocol #818407 on (DATE) in Stellar Chance. ICF version: (VERSION NUMBER) The consent was reviewed with the patient who was on their own and their questions about how the data would be used for research were answered. Patient understood the research and was open to participating, also aware they could decline to participate at any point. Then the patient signed the consent form and was given a copy.
